INCIDENT REPORT FORM

Students having any type of incident while in the laboratory or clinical setting will complete a College of Nursing Incident Report Form available from the faculty. This form describes the incident as well as follow-up treatment. This form will be used in addition to forms that may be required by the clinical agency. Students are responsible to notify faculty of any incident that occurs.

SMOKING POLICY

Linnea E. Henderson Hall has been determined to be a smoke-free environment. There is no smoking permitted in classrooms, conference rooms, hallways, lounges, entrance ways or the main office area. As a guest of health care facilities offering student experience, the student will follow the smoking policy of the health care facility.
